  • Health Information
Help us provide the best health care possible. Be sure to have information on hand, to the best of your knowledge, regarding your present symptoms, past illnesses, hospitalizations, medications and other health-related matters.
  • Health Care Proxy
It's important for the St. Francis staff to know exactly how you want to be cared for, particularly in critical moments. A Health Care Proxy documents your wishes for treatment in times when you can't express those wishes yourself. Download the Health Care Proxy form now, so that you can complete it prior to your visit.
  • Personal Items
Plan on keeping only essential items with you during your stay—a robe, slippers, pajamas, and toiletries. Any other valuables, such as jewelry or credit cards, should be sent home with a friend or relative.
  • Discharge Procedures
